Working Together to Build Stronger Pathways to Housing Stability

Housing stability plays an important role in individual health and well-being. For healthcare organizations, health plans, and community partners, connecting members with appropriate housing-related services can be an essential part of supporting their overall needs.

Advanced Healthcare Administration (AHA) provides personalized Housing Navigation services through applicable CalAIM Community Supports, helping eligible members navigate barriers to finding and maintaining stable housing.

We welcome opportunities to collaborate with organizations that share our commitment to improving access to housing-related support and strengthening the coordination of services across our communities.

Who We Work With

Addressing housing challenges requires collaboration across healthcare, housing, and community services. AHA works with and welcomes inquiries from organizations involved in supporting individuals experiencing housing instability.

Our partnership and referral network may include:

  • Medi-Cal Managed Care Plans (MCPs): Exploring opportunities to support eligible members through housing-related Community Supports.

  • Enhanced Care Management (ECM) Providers: Coordinating housing-related needs alongside broader care management efforts.

  • Hospitals & Healthcare Providers: Supporting connections between healthcare services and housing navigation.

  • Behavioral Health & Substance Use Disorder (SUD) Providers: Collaborating to address housing barriers that may affect stability and continuity of care.

  • Social Workers & Case Managers: Helping connect eligible members with appropriate Housing Navigation services.

  • Community-Based Organizations: Strengthening coordination and access to available housing-related resources.

  • Housing Providers & Property Managers: Supporting communication and coordination throughout the housing navigation and tenancy process.

Whether you're looking to establish a referral relationship, explore service coordination, or discuss a potential organizational partnership, we'd welcome the conversation.
