Helping Your Members Take the Next Step Toward Stable Housing

Advanced Healthcare Administration (AHA) works with healthcare providers, case managers, social workers, and community partners to help eligible members navigate barriers to finding and maintaining stable housing.

Through applicable CalAIM Community Supports, our team provides personalized Housing Navigation services designed around each member's individual circumstances, housing needs, and goals.

If you're working with someone who may benefit from housing-related support, review the referral eligibility criteria below to determine whether AHA may be an appropriate provider.

Who Is Eligible for a Referral?

Before referring a member to AHA, please confirm that they meet the following initial screening criteria.

The member must meet all three of the following conditions:

  • Orange County Residency: The member currently resides in Orange County, California.

  • CalOptima Health Coverage: The member has CalOptima Health.

  • Housing Status: The member is currently unhoused or will imminently lose housing within 30 days.

Additionally, the member must meet at least ONE of the following criteria:

  • Meets access criteria for Medi-Cal Specialty Mental Health Services (SMHS).

  • Meets access criteria for Drug Medi-Cal (DMC) or Drug Medi-Cal Organized Delivery System (DMC-ODS), as defined by DHCS's Community Supports Policy Guide.

  • Has one or more serious chronic physical health conditions.

  • Has one or more physical, intellectual, or developmental disabilities.

  • Is pregnant or within 12 months postpartum.

Meeting these initial screening criteria does not guarantee acceptance into services. All referrals are subject to applicable program requirements, eligibility review, available resources, and AHA's current capacity.

Housing Navigation Services Available Through AHA

AHA provides housing-related Community Supports to help eligible members navigate the process of finding and maintaining stable housing.

Housing Transition Navigation Services

Personalized support to help members navigate housing searches, applications, documentation, communication with housing providers, and other barriers to securing housing.

Housing Tenancy & Sustaining Services

Support to help members maintain stable tenancy, address housing-related challenges, and navigate resources that may help prevent housing loss.

Housing Deposits For Eligible Members

Certain housing-related deposit assistance may be available, subject to program requirements, authorization, available resources, and AHA's capacity.
